A missed appointment fee applies to all missed office visit appointments - please call twenty four hours in advance to cancel or reschedule your office visit appointment in order to avoid this fee. Missed appointment fees do not apply to lab-only appointments.

New patients must arrive at least 30 minutes early for your scheduled appointment as there will be paperwork that must be completed in-person.

 

Established patients must arrive 5-15 minutes early for your scheduled appointment in case there is paperwork that must be completed in-person.

We have updated our Arrival Time Policy regarding late appointments.

ALL patients must bring ALL MEDICATIONS, or a complete list of medications, to EVERY visit! Failure to do so may result in your appointment being canceled and incurring a missed appointment fee.
